Licensed Practical Nurse (LPN)

We are seeking a Licensed Practical Nurse to join our long-term care facility. The role involves administering medications and treatments, monitoring vital signs, and providing assistance with activities of daily living (ADLs). Clinicians will work as part of an interdisciplinary team, which includes physical therapists, occupational therapists, speech-language pathologists, case managers, social workers, and dieticians. The primary patient population includes individuals requiring long-term care, rehabilitation, and dementia support. Nursing duties include blood glucose monitoring, insulin administration, IV medications, tube feedings, wound care, and performing lab draws. The position operates in units with a majority of floor nurses being LPNs, with some RN supervision during various shifts. Requirements include BLS certification and the ability to perform basic nursing functions in a skilled nursing environment. Staff will rotate through different units as needed based on facility requirements. A clean, professional dress code is required, with no specific scrub colors mandated. All nurses participate in team care meetings for residents, assisting in care plan development as applicable.
