Occupational Therapist

$2203.20/week - Job Summary: Provide occupational therapy services to patients in a home care setting, focusing on enhancing functional independence and quality of life. Collaborate with interdisciplinary teams and families to develop personalized treatment plans. Responsibilities: Assess patients' functional abilities and identify treatment needs. Provide direct therapeutic interventions to address patient challenges. Select and implement appropriate therapeutic modalities. Communicate treatment plans and progress with team members and patients. Document evaluations, treatment goals, and patient progress accurately. Educate patients and families on treatment rationale and home exercise programs. Qualifications: Possess a valid occupational therapist license in Vermont and New Hampshire or a current unencumbered compact license. Hold a Bachelor’s Degree in Occupational Therapy or an advanced certification. Successfully pass the National Registration Exam. Demonstrate experience or familiarity with home health care preferred.
