Must be able to successfully complete all of the functions and competencies associated with this position as well as other duties as they may be assigned.Essential Job FunctionsCome to work on time, in the proper attire prepared to workMake sure all required training and certifications, licenses and insurance requirements are up to dateAdjust work hours as required for effective operation of the programProviding individuals with assistance in everyday life skillsProvide active treatment, emotional and health-related services including administering medications, assisting in breathing treatments, checking vitals to monitor overall health and assisting with medical equipment.Assist with providing basic personal care and grooming, including bathing, basic hair care and getting dressedResponsible for administrative task associated with providing support.Accurately administering medication and following and maintaining MARSImmediately report any incidents or accidents involving individuals to the Site LeaderAccurately, legibly and cohesively document all services provided to individualsMaintain a clean and safe facility and always ensure the safety and wellbeing of the individualsInvolve individuals in community and recreation-based activitiesAssist with and or complete household chores such as cleaning, laundry, cooking, vacuuming, and othersMake and accompany individuals to appointments and community eventsPerform and record physical assessments including taking and recording vital signs and medical historiesExhibit acceptable work standards and immediately correct actions that do not meet Agency standards.Ensure completion of required annual training by established deadlines.Follow Homemaker and Personal Care (HPC) electronic and paper documentation. Make sure that ISP's and Behavior Support Plans are followedReport UIR and MUI situations to Site Leader according to guidelines providedKeeping and documenting records of body and general living conditionsTransport individuals in Agency vehicles to various locations in the community. This may also include transporting individuals who do not reside at residential site.All other related tasks as assigned by management.RequirementsMinimum of a High School Diploma or GED and 1 year of work experience preferably in a care providing role, or exposure to individuals with developmental or intellectual disabilities or a combination of experience and educationBasic level of computer proficiency as well as good basic reading and writing skillsExcellent communication, interpersonal charting and record keeping skillsAbility to accept and provide direction and instruction in a professional mannerSuccessfully pass a background check that includes a BCI and FBI criminal history checkMust maintain current Med Pass and CPR/First Aid Certification; training available for CPR and Med Pass onsiteMust be a valid driver with a current Ohio Driver's License reflecting less than 6 points and proof of state minimum vehicle/general liability insurance coverage.Americans with Disabilities SpecificationsPhysical DemandsThe physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
