RN Case Manager - Home Health (On-Call & 401K)
Harbor Healthcare System
Harbor Healthcare System is looking for an experienced RN-Case Manager in Houston, Texas. This out-patient position requires coordination of patient care with an interdisciplinary team and skilled nursing care at clients' residences. The ideal candidate must be a Licensed RN in Texas with at least 2 years of Home Health experience. Reliable transportation and a clean background are also necessary.
