Transitional Case Manager

Position Summary

Provides transitional case management services to persons diagnosed with a severe and persistent mental illness or co-occurring disorders. Primary functions include: assessing case management referrals to determine immediate and/or ongoing needs and crisis interventions.

Essential Duties and Responsibilities

The essential functions include, but are not limited to the following:

• Contacts referred consumers to case management within policy guidelines to begin assessing individual needs and treatment goals.
• Manages transitional case management list.
• Arranges and coordinates identified services needed to assist consumers in securing safe housing, utilizing emergency assistance programs, making application for Missouri Health Net and other eligible entitlements. Secures appropriate releases and provides needed follow-up.
• Utilizes independent judgment to problem solve crisis situations and provides crisis interventions accordingly.
• Provides outreach as needed in order to engage referred consumers to community support services.
• Encourages and assists consumers in establishing relationships in the community through social interactions, exploring their natural supports available, and referring consumers to other resources when appropriate.
• Maintains communication with treatment teams to ensure appropriate services are in place.
• Available to other agency departments/teams for consultation on community resources, and informs others regarding latest resource information. Attend team meetings (example: assessment, medication services) if needed.
• Maintains productivity expectations per agency standards.
• Completes progress notes in AVATAR within 72 hours of services provided.
• Maintains knowledge of agency technology programs and/or systems needed to do the job and uses them effectively.
• Maintains required training and certificates.
• Attends and participates in weekly team meetings.
• Maintains a valid driver's license and reliable transportation
• Other duties and/or responsibilities may be assigned by supervisor.

Minimum Qualifications (Knowledge, Skills, and Abilities)

Education/Certifications:

• Bachelor's degree in a related field
• Active driver's license and minimum state-required auto-insurance.
• CPR/First Aid certification

Experience Required:

• 1 to 3 years minimum work-related experience required
• 3+ years minimum work-related experience preferred

Required Knowledge:

• Knowledge of behavioral health diagnosis and symptoms.
• General knowledge of community resources and/or the willingness to research as needed

Skills/Abilities:

• Strong assessment and crisis management skills
• Excellent communication skills, both written and verbal.
• The ability to use and troubleshoot basic business equipment.
• The ability to interact with a wide range of individuals.
• Strong ability to multi-task with a high level of accuracy and timeliness in project output.
• The ability to maintain a high level of client confidentiality.
• Ability to problem solve quickly and prioritize tasks for optimum efficiency.

The employee must occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this position include close vision, distance vision, and the ability to adjust focus. The noise level in the work environment is usually low to moderate.
