Provides personal care and housekeeping assistance to clients in their homes; follows the established plan of care under the direction of the client care coordinator. Provides personal hygiene and groom care to clients. Provides meals for clients' nutritional needs. Reports changes in clients' condition to client care coordinator or supervisor. Performs routine housekeeping and home management duties. Records each client's care on care plan and submits care plan in a timely manner. Assists in providing emotional support for the client and family. Assists with routine therapy exercises. Reminds client of medication regime, opens medicine containers for clients as needed. Participates in case conferences as requested. Attends required in-service sessions. Will actively participate in the performance improvement process known as H3W.

Qualifications

Preferred Education Requirement: High school diploma or equivalent

Preferred Experience: Six months experience as a Home Care Aide or CNA in a home care environment.

Required Licensure: Home Care Aide or CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services

Regular Standard Hours Per Week: 32

Schedule: Part-time (2 - 39 hours)

Shift Details: Monday thru Friday 8am to 4:30pm with 1 weekend per month and 1 major and 1 minor holiday per year. Covering the Torrington, CT region and surrounding towns
