About This Role As a Care Coordinator at Cleveland Clinic Fairview Hospital, you will work collaboratively with a multidisciplinary care team to provide coordination of care and disease management for high‑risk patients, focusing on optimal outcomes and wellness. Highlights Pay: $63,250.00 - $96,467.50 Location: Cleveland, OH Facility: Cleveland Clinic Fairview Hospital Department/Unit: Moll Center Schedule: Full‑time, Day shift What You'll Do Work collaboratively with a multidisciplinary care team to develop goals and plan interventions. Provide care and disease management coordination. Identify patients with ongoing coordination needs and conduct targeted outreach. Conduct comprehensive clinical assessments including medical, behavioral, and social needs. Inform and work with patients and families regarding care coordination. Monitor patient compliance with care plans and perform reassessments. Serve as a liaison and advocate for patients and families. Assist in managing transitions of care across settings. Identify barriers, facilitate solutions, and connect to community resources.
