Day Hab without Walls!

We have positions in Nassau and Suffolk Counties!

Use of a smart phone is a key factor to this position for clocking in digitally and for encrypted communications.

Light documentation is required for daily (checklist of individual's goals) and monthly (summary of goals worked on.)

We offer monthly mileage reimbursement.

Starting rate: $19.00. Bi-weekly pay period.

FLEXIBLE SCHEDULING!

20-30 hours weekly (Part-Time) includes paid holiday, vacation, and sick time off.

If you are looking for Full-Time (35-40 hours weekly) we are open to that as well.

Job Description:

Facilitate an individual's independence, individualization, inclusion and productivity (otherwise known as the 3 IP's)

Complete daily checklists and monthly summaries based on the valued outcomes on the Day Habilitation Plan

Support and advocate for all individual's within the community

Create meaningful activities for individuals within their community based on their personal goals

Performs all other duties as assigned

Onboarding Process:

Interview.

Job Offer.

OPWDD Fingerprint and Background Clearance.

Proof of Valid NYS Drivers License.

New Hire Orientation.
