Nurse Practitioner

We are seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equitable, value-based primary care to Medicare adults. Responsibilities providing comprehensive primary care conducting office visits for routine and acute issues administering wellness visits and health assessments coordinating care with specialists and testing facilities leading population health initiatives assisting with phone triage educating patients participating in promotional activities and conducting home visits as needed The role involves working within a collaborative outpatient team, reporting to the Center Medical Director, and focusing on primary care, care coordination, and population health.
