Role Description This is a full-time, on-site role for a Case Manager located in Lake Zurich, IL. The Case Manager will be responsible for coordinating client care plans, conducting assessments, and collaborating with healthcare teams to ensure individualized and efficient services. Responsibilities include monitoring client progress, maintaining accurate records, and providing resources for additional support. This role involves regular communication with clients, families, and professionals to advocate for optimal care and outcomes. Qualifications Experience in Case Management, Care Coordination, and Client Advocacy Experience working with individuals with special needs (preferred) Exceptional Communication, Interpersonal, and Problem-Solving Skills Proficiency in Assessment, Planning, and Resource Management Ability to work collaboratively in a healthcare or social services environment Strong organizational skills and attention to detail Knowledge of healthcare services, community resources, and state regulations Bachelor’s degree in Social Work, Psychology, or a related field (required) Previous experience working in a healthcare setting or case management (preferred) #J-18808-Ljbffr Mtsj
