We are seeking a compassionate, detail oriented MDS Coordinator to join our clinical leadership team. This role is responsible for coordinating resident care planning, completing MDS assessments, ensuring regulatory compliance, and supporting exceptional resident-centered care.

Position Summary

The MDS Coordinator oversees the coordination of care through comprehensive care planning and timely completion of Minimum Data Set (MDS) assessments for residents. This position collaborates closely with nursing staff, interdisciplinary team members, residents, and families to ensure individualized care plans are developed, implemented, and maintained.

The MDS Coordinator serves as a key resource in maintaining quality standards, regulatory compliance, and resident satisfaction while supporting the overall mission of Parc Provence.

Key Responsibilities

Care Planning & MDS Management

• Coordinate care conferences and schedule meetings with residents' families.
• Develop, implement, and update individualized resident care plans.
• Complete and maintain MDS assessments in the electronic medical record system.
• Ensure care plans and MDS documentation are accurate, complete, and submitted timely.
• Monitor resident condition changes and communicate updates to nursing leadership.
• Obtain required interdisciplinary team signatures and participation in the care planning process.

Clinical & Regulatory Support

• Maintain compliance with state regulations, facility policies, and clinical standards.
• Assist with staffing coverage as needed, including Charge Nurse responsibilities.
• Complete recaps and clinical reviews for assigned households.
• Report safety and maintenance concerns promptly.
• Participate in Manager-on-Duty (MOD) rotation.
• Support quality assurance and clinical documentation initiatives.

Employee Health & Education

• Administer and track TB testing and employee health requirements.
• Conduct health assessments for new employees.
• Present nursing-related topics during new employee orientation.
• Participate in ongoing staff education and training programs.

Qualifications

Required

• Active Missouri nursing license (LPN or RN).
• High school diploma or equivalent.
• Ability to read, write, and communicate effectively in English.
• Strong organizational, documentation, and communication skills.
• Ability to collaborate effectively with residents, families, physicians, and interdisciplinary team members.

Preferred

• Previous MDS experience.
• Experience in long-term care, skilled nursing, rehabilitation, memory care, or senior living environments.
• Experience developing and managing resident care plans.

Schedule & Availability

• Full-Time
• Participation in MOD rotation required.
• Must be available to work weekends, holidays, and additional hours as needed to support resident care and community operations.
