Clinical Nurse Care Manager I

Job Summary The Clinical Nurse Care Manager (CNCM I) position supports the healthcare team in transitional care by being a liaison between patients, their families, doctors, and health care providers. CNCM I facilitates transitional care, aiming to enhance the quality of clinical outcomes and patient satisfaction. CNCM I helps drive change by identifying areas where performance improvement is needed, such as day-to-day workflow, process improvements, and patient satisfaction. CNCM I is accountable for patient’s caseload and plans effectively to meet patient needs across the continuum of care, provides family support, manages length of stay, and promotes efficient utilization of resources. CNCM I advocates for patients and coordinates their care and works with other health care professionals to determine patient goals and create short- and long-term plans. CNCM I participates in quality improvement activities, exemplifies professionalism, and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members. Minimum Qualifications Graduation from an accredited school of Nursing with a Bachelor's degree required. Registered Nurse licensed to practice nursing in the State of Texas. Related specialty certification preferred: Basic Life Support. Three (3) years experience as RN in appropriate area assigned required; one (1) year experience in Case Management, Quality Management, and Coding preferred. One (1) year leadership/clinical experience preferred. Communication Skills: Above Average Verbal (Heavy Public Contact), Exceptional Verbal (e.g., Public Speaking), Writing/Correspondence, Writing/Reports. Proficiencies: MS Word, PC. Job Attributes Knowledge/Skills/Abilities: Analytical, Medical Terms, Mathematics. Work Schedule: Weekends, Holidays, Flexible, Travel, On Call.
