What You’ll Do

• Provide hands-on nursing care focused on comfort and quality of life
• Administer medications and monitor patient responses
• Observe, document, and report changes in condition
• Collaborate with RN Case Managers and interdisciplinary team
• Support patients and families with compassionate communication
• Follow individualized care plans and ensure HIPAA compliance

What We’re Looking For

• Active Ohio LPN license (required)
• 1+ year of nursing experience (hospice or home health required)
• Strong clinical and documentation skills
• Reliable, compassionate, and patient-focused
• Ability to work independently and as part of a team
