Description

  • Assess and evaluate patients in either inpatient or outpatient settings and create personalized treatment plans
  • Deliver physical therapy interventions concentrating on mobility, strength, balance, and overall function
  • Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes
  • Guide patients and their families on therapeutic goals, discharge processes, and home exercise routines
  • Maintain detailed documentation of evaluations, treatments, and patient progress as per facility and regulatory standards
  • Participate in team rounds and interdisciplinary meetings to ensure cohesive patient care

Education

  • Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

  • Current state licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional specialized certifications (e.g., neurological, geriatric) are advantageous but not mandatory

Skills

  • Prior experience is highly preferred
  • Ability to work with patients facing complex medical and mobility challenges

  • Proficient with digital charting and clinical documentation systems
