Description

• Conduct patient evaluations in both inpatient and outpatient environments and craft personalized treatment plans.
• Implement physical therapy interventions focusing on equilibrium, mobility, strength, and overall function.
• Work collaboratively with nurses, physicians, case managers, and other rehabilitation specialists to ensure optimal outcomes.
• Educate patients and their families on therapeutic objectives, discharge processes, and home exercise programs.
• Maintain documentation of evaluations, treatments, and patient progress adhering to regulatory and facility guidelines.
• Engage in interdisciplinary care team rounds and conferences to guarantee well-coordinated care.

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification obtained from an accredited institution.

Certifications

• State licensure as a Licensed Physical Therapist
• CPR certification preferred.
• Specific certifications (e.g., specializing in geriatrics, neurology) are valuable but not essential.

• Relevant experience is highly preferred.
