Responsibilities Conduct initial and ongoing assessments of patients' and residents' physical, emotional, and social needs Develop and implement personalized care plans to address their needs and goals Collaborate with interdisciplinary teams to ensure comprehensive care delivery Provide counseling and support to patients and residents, as well as their families and caregivers Facilitate communication among patients, families, and healthcare providers Identify and address social determinants of health that impact patients' and residents' well-being Requirements Licensed Clinical Social Worker (LCSW) in the state of Massachusetts Previous experience in Long-Term Care (LTC) is preferred Strong communication, interpersonal, and problem-solving skills Ability to work independently and as part of a team Compassionate and empathetic approach to patient care What We Offer (Please note that benefits information is not provided.
