Physical Therapist – General, Ukiah, California. Travel assignment starting 09/28/2026 for 13 weeks. Weekly pay per week: $2,040–$2,172. Guaranteed hours: 40 per week. Shift: 5x8 days (8:00 AM–4:30 PM). Setting: Short Term Acute Care Hospital.

Responsibilities
- Conduct evaluations and ongoing assessments to guide treatment plans for physical, social, and functional goals.
- Develop and implement individualized PT treatment plans; adjust as progress is made.
- Deliver therapy interventions using evidence-based protocols and techniques to address functional deficits.
- Collaborate with an interdisciplinary care team and co-lead patient groups as appropriate.
- Establish discharge plans and communicate recommendations to patients, families, and the care team.
- Complete comprehensive clinical documentation within established deadlines; supervise assigned assistants as needed.

Requirements
- Doctor of Physical Therapy (DPT) degree; required if license obtained after January 2016 or foreign equivalent.
- Current Physical Therapy licensure in the state of practice.
- Two years of Physical Therapy experience preferred.
- Licensure/certifications: Physical Therapy licensure in state of practice; Basic Life Support (BLS) or CPR certification required per policy.
- EPIC EMR experience required.
- Pelvic floor therapy experience with PRPC or WCS certification, or actively working toward certification, preferred.
- Ability to work in a short-term acute care setting and meet facility credentialing requirements.
