Position: Case Manager Reports to: Case Manager Supervisor Location: Chicago, Illinois Organization Background The Precious Blood Ministry of Reconciliation (PBMR) mission is: Rooted in the spirituality of the Precious Blood, we restore human dignity through hospitality, hope, and healing. We work as agents of reconciliation to: Build relationships among youth and families impacted by violence and/or conflict Create safe spaces where people can experience radical hospitality, hope, and healing, and Promote a restorative approach to conflict and build a sense of community. For more information, please visit Position Summary The Case Manager works with a caseload of participants to assist them in their journey towards becoming a productive, active member of the community. The Case Manager will work with individuals who are active in the streets as identified by the outreach workers, will participate actively with Outreach team and have a demonstrated interest in taking action for change. Case management for our participants includes, but is not limited to, support related to employment and education, life skills training, crisis intervention, relationship training, advocacy and support with involvement with justice system, and assessment and referrals to needed resources. Services will be provided in individual, family, and group context. Essential Functions Accomplishes participants' care by assessing individual needs; providing direct services and engaging in ancillary activities; participating in an interdisciplinary team of other PBMR staff, IPP staff, and community partners. The essential functions of the position include, but are not limited to, the following: Identifies and develops relationships with at promise individuals within target community Engages participants in interviews to assess needs and determine need for additional services and Refer and coordinates internal and external services being provided to participant; identifies and arranges resources Monitors cases by verifying participants' attendance; observing and evaluating activities and responses; advocating for needed services; identifying and connecting to additional resources; intervening in crises; and providing personal support Supports and teaches as necessary various job readiness workshops to individuals and groups, including computer skills, communication, critical thinking, self-awareness, conflict mediation, financial literacy, job search and application techniques, resume and cover letter development, interview skills, etc. Maintains participants' records by inputting case notes, assessments, and events daily in online data system Communicates participants' progress by engaging in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to team and family, and identifying program influences Prepares participants' exit strategy by reviewing goals and individualized plans, coordinating discharge and post-discharge requirements, orienting and training family members, and providing resources Develops partnerships with resources that will strengthen supports for participants and holds partners accountable as needed Duties as assigned by supervisor Case Managers are mandated reporters and are expected to work at high level of team engagement Qualifications Ability to live out of a restorative justice philosophy and use the circle process when needed Skilled in building healthy relationships, maintaining boundaries and valuing confidentiality Experience in working with a diverse population and diverse personalities Willingness to be a good steward of resources (financial, etc.) Ability to effectively communicate through written, verbal, and electronic methods (including various social media applications, text, email, and phone calls) Ability to be a self-starter, problem-solver and dreamer of possibilities Conduct oneself in a professional, warm and approachable manner Organized and able to meet deadlines and multitask Competent using Microsoft Office 365 suite Able to work in a fast-paced environment Schedule/Travel Monday through Friday with the possibility of working on occasional evenings and weekends with sporadic local travel.
