Social Worker

If you're a social worker who wants your caseload to mean something, Bella Home Health / Hospice is where that work happens. We're hiring a Social Worker to join our interdisciplinary hospice team. This is a full-scope clinical role: psychosocial assessments, counseling, community resource coordination, and direct leadership of our Bereavement and Volunteer programs. WHAT YOU'LL DO You'll be the emotional and logistical anchor for patients and families navigating end-of-life care. That means conducting psychosocial assessments, providing short-term counseling, and serving as the link between patients, families, physicians, hospitals, and community organizations. You'll make home visits, participate in on-call rotation, and keep documentation accurate - because good paperwork protects patients and supports everyone on the care team. You'll also own two programs that carry real weight: Bereavement and Volunteer Coordination. These aren't add-ons to the role. They're a meaningful part of it. Beyond direct care and program management, you'll stay current on Medicare/Medicaid standards, maintain community resource relationships, and participate actively in interdisciplinary team meetings and continuing education. WHAT WE'RE LOOKING FOR We're looking for an MSW from a CSWE-accredited school, or a BSW with supervision by a qualified MSW per CMS hospice conditions of participation. You'll need at least one year of social work experience, a current state social work license, CPR certification, and a valid driver's license with reliable transportation and current auto liability insurance. Hospice, home health, or palliative care experience is strongly preferred, and any background in bereavement or volunteer coordination makes you stand out. More than credentials, we're looking for someone who is steady. This work is emotionally demanding. The right person leads with clarity and compassion, manages a caseload and two programs without dropping the ball, communicates well in writing and in person, and shows up consistently for patients and families. You know how to navigate community resources and follow through. You're a genuine team player in an interdisciplinary setting, and you don't cut corners on Medicare/Medicaid documentation.
