Acute Care Case Manager - Discharge & Care Coordination

Paradise Valley Hospital, part of Prime Healthcare, is seeking a per diem Case Manager to oversee quality and resource management for admitted patients across the care continuum. You will advocate for cost containment and coordinate multidisciplinary teams to ensure a seamless delivery of care. Required BS W or related bachelor’s degree and at least one year in case management, discharge planning, or nursing management, plus basic to intermediate computer skills. #J-18808-Ljbffr Prime Healthcare
