Description

American Traveler is hiring an LPN for a home health assignment requiring HCHB EMR proficiency and 1+ year of recent experience (license required if applicable).

Details

• Home health setting serving a geriatric patient population
• Caseload of approximately 5-7 patients per day
• Monday-Friday schedule with flexibility to work weekends as needed
• Territory of up to a 60-mile radius from the hub office, requiring flexibility across the territory
• No on-call required, though an on-call rotation may be discussed during interview

Requirements

• License required if applicable
• Minimum 1 year of recent relevant home health experience
• BLS certification required
• Proficiency in HCHB (Homecare Homebase) EMR required
• OASIS experience required for staff completing Start of Care (SOC) visits
• Active driver's license and personal transportation required
• Skills including medication administration, IV therapy/phlebotomy, wound care/wound vac, NG/G tube care, and injections (IM & SQ)

Additional Information

• Provide skilled nursing visits including resumption of care and routine visits for geriatric home health patients
• Maintain 30 productivity points weekly as required
• Timely documentation and communication with the care team required
• Scrubs required in any color, no jeans permitted
• iPad provided for documentation; personal cell phone not required
• Mileage reimbursed at the IRS standard rate
• First-time travelers will not be considered
• Locals accepted at the travel rate
• Travel pairs accepted, though each traveler is assigned an individual territory and must have their own transportation
• Former Compassus employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
• Requested time off (RTO) must be submitted at the time of application
• Monday orientation required, including completion of billable modules
• Hospice/Home Health checklist required
