Independent In-Home OT Evaluator - Contract

Jukebox Health, Inc. is seeking an Occupational Therapist to support older adults and individuals with disabilities at home. The role centers on in-home assessments, evidence-based home modification recommendations, and fall prevention education for clients and caregivers. The contract position requires an active OT license, 2+ years of clinical experience, and reliable transportation.
