Responsibilities:- Oncall hospice visits on weekends
- Provide accurate and timely documentation
- Assess and provide patient and family/caregiver education
- Perform other duties as assigned by the registered nurse
- Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc. Qualifications:
- Demonstrated compassion and commitment to patient centered care
- Active LVN license in the state of California
- Valid California driver’s license and reliable transportation
- Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care
- Prior experience in nursing or related medical fields
- Experience as an on call LVN runner preferred
- Weekends (Friday Saturday and Sunday) 6:00 pm 8:00 am Service Coverage:
- Inland Empire
