Job Title: RN Visit Nurse – Home Health, 1:1 Care | Strong Team Culture + $4K Bonus

Location: Greater Twin Cities Metro Area and surrounding Minnesota service area

Schedule: Full-Time with On-Call and Weekend Rotation

Compensation: $75,000 – $100,000 per year plus $4,000 Sign-On Bonus

What You’ll Do

• Provide skilled nursing visits in patient homes and care settings.

• Deliver hands-on care including wound care, medication management, and assessments .

• Build consistent relationships with patients and families.

• Coordinate care with physicians, therapists, and interdisciplinary teams.

• Educate patients and caregivers on care plans and recovery goals.

• Complete accurate, timely documentation .

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.

• Experience in home health, med-surg, rehab, or clinical nursing .

• Ability to work independently while staying connected to a team.

• Strong communication and patient education skills.

• Reliable transportation and willingness to travel locally.
