Nurse Practitioner (NP) – Community-Based Care

Location: Brooklyn, NY
Employment Type: Full-Time
Schedule: Monday–Friday

Salary Range: $98,446 - $200,539 Annually (DOE)**

Job Description:

We are seeking a licensed Nurse Practitioner (NP) in New York State to provide in-person patient assessments and ongoing care management throughout assigned borough areas of New York City. This is a community-based role that requires strong clinical judgment, autonomy, and excellent organizational skills.

The NP will manage a structured daily schedule and a defined patient caseload , delivering high-quality care directly in community settings.

Responsibilities:

• Conduct in-person assessments for up to 12 patients per day
• Develop and implement individualized care plans
• Manage a caseload of approximately 200–300 patients
• Travel between designated meeting points within assigned boroughs (no home pickups required)
• Begin daily visits at 8:30 AM and continue until daily patient quota is met
• Monitor chronic conditions and provide ongoing treatment management
• Maintain accurate and timely documentation for all patient encounters
• Collaborate with interdisciplinary care teams to ensure continuity of care

Qualifications:

• Active and unrestricted Nurse Practitioner (NP) license in New York State
• Strong clinical assessment and diagnostic skills
• Ability to work independently in a community-based setting
• Excellent communication and interpersonal skills
• Strong time management and organizational abilities
• Experience in primary care, geriatrics, or chronic disease management preferred

Preferred Experience:

• Community-based or field-based clinical care
• Care coordination for complex or high-risk patients
• Experience with electronic medical record (EMR) systems
