Provide therapeutic interventions to improve strength, mobility, balance, coordination, and functional independence. Help patients safely achieve their mobility and functional goals in their home environment. Educate patients and caregivers on exercises, mobility strategies, fall prevention, and appropriate use of equipment. Collaborate with patients, families, physicians, and interdisciplinary team members to coordinate care and achieve optimal outcomes. Monitor patient progress and adjust treatment plans as needed. Complete timely and accurate clinical documentation in accordance with company and regulatory requirements. Maintain current clinical knowledge through continuing education and professional development. What We're Looking For Current Massachusetts Physical Therapist license. Home health or outpatient experience preferred. Strong clinical assessment and treatment skills. Excellent communication and interpersonal skills. Ability to work independently and effectively manage a patient caseload. Strong organizational and time-management skills. Valid driver's license and reliable transportation.
