Field-Based PPP Support Counselor - Budget & Care Planning

Horizon Blue Cross Blue Shield of New Jersey is seeking a Self-Direction PPP Support Counselor (Field Based) to conduct quarterly in-person evaluations for members enrolled in the Personal Preference Program in Hudson County, NJ. The role includes budgeting, back-up planning, capacity forecasting, and real-time monitoring. You will review monthly expenditures, update budgets, maintain the member’s records, identify fraud, and connect members with resources. #J-18808-Ljbffr Horizon Blue
