Key Responsibilities\n

• Collaborate closely with a multidisciplinary healthcare team to create and implement comprehensive treatment plans, including detailed discharge plans based on thorough psychosocial assessments.

• Establish strong relationships within community resource groups, representing Redesign Counseling Hub during meetings and committees to advocate for the social needs of our patients.
\n
• Provide crisis intervention and counseling to clients, actively assessing and treating a broad spectrum of emotional and behavioral issues.

• Communicate effectively with referring physicians regarding patient progress, challenges, and needs, ensuring an integrated approach to client care.

• Maintain timely and accurate clinical documentation including progress notes, treatment updates, and other required records in compliance with departmental policies.

The role focuses on a diverse patient population, addressing mental health issues that range from anxiety and depression to more complex behavioral disorders.
