Serenity Hospice is seeking a compassionate and dedicated Hospice Social Worker to join our team serving the Wyoming Valley, Pennsylvania area. This is a rewarding opportunity to make a meaningful impact by supporting patients and their families during one of life’s most important journeys.

Position Summary:
The Hospice Social Worker provides emotional, psychosocial, and practical support to patients and families, helping them navigate end-of-life care with dignity and comfort.

Responsibilities:

• Provide psychosocial assessments and ongoing support to patients and families

• Assist with care planning as part of the interdisciplinary team

• Connect patients and families with community resources

• Offer counseling related to end-of-life issues, grief, and coping

• Maintain accurate and timely documentation

Qualifications:

• Master’s Degree in Social Work (MSW) required

• Current Pennsylvania licensure (LSW/LCSW preferred)

• Previous hospice or healthcare experience preferred

• Strong communication and organizational skills

• Valid driver’s license and reliable transportation
