RN - Care Manager

Care Manager Registered Nurse (RN)The Care Manager Registered Nurse (RN) works collaboratively with interdisciplinary and healthcare team members, both internal and external to the organization, to facilitate patient care through effective utilization and monitoring of healthcare resources. This role assumes a leadership position to achieve safe discharge planning alongside desired clinical and financial outcomes.Key ResponsibilitiesCoordinate care and services throughout the acute care episode and across the continuum.Effectively utilize and monitor healthcare resources.Assist in safe discharge planning.Collaborate with healthcare team members to enhance patient care.QualificationsEducation: Associate degree in Nursing is required.Certifications: AHA BLS or Red Cross certification required.Licenses: Registered Nurse license in the state of Alabama or a compact license (No temporary licenses accepted).Experience: Minimum of 2 years of RN experience is required. A BSN or a related bachelor's degree in a healthcare field is preferred. Relevant experience in utilization and/or case management is also preferred.General InformationThe Care Manager Registered Nurse must be willing to float to other units and hospitals. The position will accept either 36 or 48 hours per week, with guaranteed hours of 36 per week.
