Position Overview

• Schedule: Full-Time, 40 Hours Per Week
• Shift: Day Shift
• Days: Variable Schedule, Including Weekends
• Assignment Length: Long-Term Contract

What You'll Do

• Serve as a trusted resource and advocate for patients and families
• Conduct outreach and community engagement activities
• Assist patients in navigating healthcare systems and available services
• Coordinate care between primary care providers, specialty providers, and community organizations
• Support case management activities and follow-up services
• Connect patients with critical resources including:
• Food assistance
• Housing resources
• Financial support programs
• Transportation services
• Community-based services
• Educate patients on available healthcare and social service programs
• Help reduce barriers to care and improve patient access to services
• Participate in community events and outreach initiatives
• Maintain professionalism and compassion while serving diverse populations

Required Qualifications

• Passion for serving underserved and low-income communities
• Desire to work in a community-based healthcare setting
• Ability to build trust and establish positive relationships with patients
• Strong communication and interpersonal skills
• Ability to work independently and collaboratively within a team environment
• Experience coordinating services and community resources preferred

Orientation & Training

• First week includes required onsite orientation and training
• Orientation begins at the Fremont Clinic
• Attendance at orientation is mandatory
• Epic and role-specific training provided
• Additional onboarding requirements completed during orientation week

• Total Years of Relevant Experience
• Completed Screening Questionnaire
• Availability and Requested Time Off for the Next 90 Days
• No time off permitted during the first two weeks of assignment

Compensation

• Local Pay Rate: $25.00 per hour
• Overtime Pay Available
• Full-Time Hours
