Join our client's care team as a Home Health Physical Therapy Assistant, providing compassionate support to patients in their homes. This role offers a supportive environment with mentorship, flexible scheduling, and opportunities for career growth. You will help manage and deliver patient care plans while working closely with a team dedicated to outstanding care.
Key Responsibilities:
- Collaborate to provide management and delivery of patient care plans
- Meet critical deadlines for charting
- Self-manage time and schedule effectively
- Travel Ability to spend approximately 20% of the day driving to/from patient locations
Schedule Monday - Friday, 8:00a - 5:00p (no weekends or holidays)
