RN Case Manager - Home Health Agency - PRN or Contractor / As Needed | Indianapolis, IN

Pediatric Nursing Specialists of Indiana, a Home Care Network (HCN) agency, is seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (PRN / As Needed) to join our agency and serve existing and returning clients in the greater Indianapolis and Metro areas.

This position requires travel to multiple counties but offers a consistent schedule to fit your personal / family commitments (no overnight shifts or overnight travel). This is a PRN position with a long-term employment opportunity that can be built to fit your schedule (travel up to 2 hours is required).

Compensation: Hourly Wage: $35 - $40/hour

Responsibilities/Requirements of an RN Case Manager / Clinical Nurse Supervisor include:

• RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
• RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
• RN to Document/Communicate/Coordinate patient status/change in condition to care team.
• RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
• Willing to travel surrounding counties.
• Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications:

• Graduate of an accredited School of Nursing.
• Minimum of one (1) year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Indiana.
• Minimum of one (1) year of experience in a home health or acute care setting or equivalent experience. 1+ years of paid home health / in-home care experience is also required. 2+ years of both are preferred.
• Minimum of one (1) year Experience in case management is required.
• CPR and First Aide Certifications required (existing or can be obtained prior to hire date through our agency or accredited third parties).
• Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
• Strong background in nursing, patient care, and confidentiality.
• Strong attendance and dependability track record with previous employers.
• Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
• Familiarity with HIPAA regulations and medical terminology.
• Current Driver's License and reliable transportation.
