Nurse Practitioner

Schedule: Monday-Friday, 8:00AM-5:00PM EST

Daily Visits: 5-7 per day

Position Summary

We are seeking a dedicated Nurse Practitioner or Physician Assistant to provide comprehensive, longitudinal primary care services to patients in their homes and community-based settings, including assisted living facilities, skilled nursing facilities, and group homes. This role is vital in improving patient outcomes, supporting continuity of care, and delivering high-quality, compassionate healthcare.

Essential Functions

Conduct in-home and community-based patient assessments, including medical history, physical exams, and medication reviews.
Complete thorough evaluations of patient health status, including risk assessments.
Diagnose conditions and develop individualized treatment plans aligned with evidence-based guidelines.
Manage chronic conditions such as diabetes, hypertension, and cardiovascular disease.
Provide preventive care, including screenings, immunizations, and lifestyle counseling.
Educate patients on disease management and healthy living strategies.
Serve as the patient's primary care provider of record .
Maintain accurate and timely EMR documentation (within 48 hours of visits).
Coordinate care with physicians, nurses, and other healthcare professionals.
Conduct follow-up visits to evaluate and adjust treatment plans.
Participate in quality improvement initiatives and adhere to healthcare standards.
Participate in after-hours on-call responsibilities as needed.
Ensure reliable transportation to scheduled patient visits.
Adhere to all policies, procedures, and compliance guidelines.
Perform other duties as assigned.

Required Qualifications

Active, unrestricted NP or PA licensure in the state of practice.
Certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant .
Ability to obtain autonomous practice status within required state timelines.
Preferred: 2+ years of primary care or longitudinal care experience.
Active DEA license .
Current BLS certification for healthcare providers.
Strong clinical assessment and diagnostic abilities.
Proficiency with EMR systems.
Valid proof of sufficient auto insurance and reliable transportation.
Valid driver's license (minimum 3 years without restrictions; no DUIs or felony driving convictions in the last 7 years).
Ability to successfully pass all required background checks and screenings.
Arizona-specific: Minimum 1 year of experience in primary care or a related field.

HEDIS/STAR measures experience
Home-based care experience
Knowledge of HCC documentation

Physical Requirements & Working Conditions

Ability to lift and carry up to 20 lbs.
Ability to sit, stand, bend, kneel, and navigate various home environments.
Ability to remain stationary for extended periods and travel between patient sites.
Ability to interpret data, solve problems, make informed decisions, and plan effectively.
Work performed primarily in patient homes, with exposure to variable environmental conditions.

Competencies & Skills

Self-motivated and able to work independently.
Strong computer proficiency, particularly with EMRs.
Understanding of CMS and ACA risk adjustment models.
Strong verbal and written communication skills.
Ability to demonstrate empathy and maintain patient trust.
Strong decision-making skills while adhering to policies and clinical guidelines.
Ability to adapt to changing conditions and priorities.
Positive, collaborative approach with patients and clinical teams.
Ability to identify, anticipate, and resolve concerns for members, providers, and health plans.
Strong clinical abilities for performing in-home assessments.
