Responsibilities
- Deliver physical therapy interventions under the supervision of a licensed PT to support mobility, strength, balance, and functional independence.
- Implement treatment plans with patient-specific exercises, gait training, transfers, ROM, and therapeutic modalities as appropriate.
- Monitor patient progress, document outcomes, and communicate changes to the supervising PT and care team.
- Ensure safety during therapy sessions, maintain infection control, and follow all applicable standards and policies.
- Collaborate with nursing and therapy staff to support comprehensive discharge planning and continuity of care.
- Complete timely and accurate documentation in the assigned rehab software and coordinate with the broader care team for scheduling and case management.

Requirements
- Active California Physical Therapist Assistant license
- SNF (skilled nursing facility) experience preferred
- NetHealth (Rehab Optima) knowledge preferred
- Ability to work the listed dayshift in a skilled nursing care setting
- Willingness to travel as assigned and to comply with program guidelines
