Licensed Practical Nurse

Sevita is hiring a Licensed Practical Nurse for our Adult Day Center, supporting adults with medical and behavioral health needs in a community-based setting. The LPN will provide direct nursing care, administer medications, monitor vital signs, and support individualized care plans in coordination with RNs and interdisciplinary teams. Responsibilities include documenting care, assisting with daily living activities, responding to health changes, and educating participants and families. You will uphold safety and regulatory standards while promoting dignity, independence, and engagement in day program activities in a compassionate, team-focused environment.
