Demonstrate proactive leadership in the care management process, including planning, facilitating, advocating, educating, monitoring, evaluating and measuring outcomes for each patient population. Commit to teamwork, assume responsibility and accountability for own duties and performance. Participate in the health system’s quality improvement initiatives, implementing disease management processes, performance improvement, quality and outcome management. Qualifications Valid Louisiana nursing license (RN or LPN). Prefer at least 5 years of clinical experience in an acute care setting, preferably with management experience; utilization and/or case management experience desirable.
