RN – Adult Care / Home Care Nurse

Location: Manhattan, NY

Type: Contract | 13 Weeks

Pay: $3,000 – $3,200 Weekly

Shift & Schedule: Day Shift | 5x8 | 40 Hours Weekly

Position Overview

We are seeking an experienced Registered Nurse (RN) – Adult Care / Home Care Nurse to provide high-quality nursing services to adult patients within the New York City home care setting. The RN will deliver comprehensive patient-centered care, conduct clinical assessments, administer treatments, and collaborate with interdisciplinary healthcare teams to ensure continuity of care and positive patient outcomes.

Responsibilities

• Assess, plan, implement, and evaluate individualized patient care plans.

• Provide direct nursing care to adult home care patients.

• Administer prescribed medications and treatments.

• Perform wound care, IV therapy, catheter care, and post-operative nursing services.

• Monitor patient conditions and report significant changes appropriately.

• Educate patients and families regarding disease management and home care instructions.

• Maintain accurate clinical documentation and patient records.

• Collaborate with physicians, therapists, case managers, and interdisciplinary teams.

• Participate in discharge planning and continuity of care initiatives.

• Follow infection prevention standards and nursing best practices.

• Complete OASIS documentation and utilize electronic medical records as required.

Required Qualifications

• Associate Degree in Nursing (ADN/ASN) or higher.

• Active New York State RN License.

• Current AHA BLS Certification.

• Minimum 2 years of recent Medical-Surgical nursing experience.

• Previous Home Care nursing experience.

• OASIS documentation experience required.

• EPIC EMR experience preferred.

• Valid Driver's License.

• COVID Vaccination Documentation.

Preferred Qualifications

• Prior NYC Home Care experience.

• Strong case management and patient education skills.

• Experience managing complex adult patient populations.
