RN - Case Manager

Job OpportunityThis position requires extensive experience with the Epic charting system, specifically in a case management role within an acute care hospital setting. Candidates should be prepared to float between two locations as needed.LocationsPetaluma, CAHealdsburg, CAWork ScheduleThe work schedule may vary week to week, with potential assignments between the two locations. Expect to work every other weekend with a mix of weekday and weekend shifts. Time off requests must be submitted in advance.Requirements & QualificationsMinimum of 2 years of charge experience requiredExperience with the Epic charting system is mandatoryTravel experience is requiredSkills & ExpertiseKnowledge of acute hospital admission and discharge planningFamiliar with CMS guidelines and HIPAA regulationsExperience with care coordination and InterQual criteriaAbility to work effectively in an interdisciplinary team, including physical therapy, respiratory services, and social servicesPatient Care DetailsThis role will focus on adults and geriatric patients, with a daily census of 30 to 35.
