Patient Services RepresentativeThe Patient Services Representative is responsible for providing high-quality customer service to patients, co-workers, and all visitors to CHC. Pre- and post-visit planning, patient outreach, panel management, coordination and facilitation of home visits, efficiently managing clinical patient flow including patient registration, coordinating with clinical staff, answering patient questions, and/or referring patients to other CHC services or departments.
