A mission-driven Federally Qualified Health Center (FQHC) in Connecticut is seeking a Primary Care Physician (MD or DO) to join its outpatient clinical team. This role provides the opportunity to deliver comprehensive, evidence-based primary care to patients across the lifespan while working within a collaborative, multidisciplinary care model.

Reporting to the Medical Director, this physician will provide direct patient care, support coordinated and integrated services, and play a meaningful role in training future family physicians to serve diverse, multicultural, and underserved populations. Precepting opportunities are also available for those who are interested.

\n

 

\n

Job Responsibilities

\n

\n
• Provide comprehensive outpatient primary care services for patients of all ages, from pediatrics through geriatrics
\n
• Conduct patient evaluations, including medical histories, physical exams, diagnostic testing, and follow-up care
\n
• Diagnose and manage acute, chronic, and complex medical conditions
\n
• Prescribe medications and treatment plans in accordance with Connecticut state regulations and clinical guidelines
\n
• Educate patients on disease prevention, health maintenance, diet, activity, and lifestyle modifications
\n
• Refer patients to specialists or higher levels of care as clinically appropriate
\n
• Document patient encounters thoroughly and accurately in the medical record
\n
• Participate in quality improvement initiatives, utilization reviews, and quality assurance activities
\n
• Collaborate with clinical and operational teams to support efficient, coordinated patient care
\n
• Participate in on-call rotation as required
\n
• Support Accountable Care Organization (ACO) activities and population health initiatives
\n
• Contribute to the training and mentorship of future family physicians
\n

\n

Qualifications

\n

\n
• MD or DO from an accredited medical school
\n
• Completion of an accredited residency program in an appropriate specialty
\n
• Board Certified or Board Eligible preferred
\n
• Active, unrestricted Connecticut medical license
\n
• Valid federal and state controlled substance licenses
\n
• Experience in an outpatient primary care or community health setting preferred
\n
• Strong interest in serving low-income, multicultural, and underserved patient populations
\n
• Ability to work effectively within a multidisciplinary team environment
\n
• Working knowledge of ICD-10, CPT, and HCPCS coding preferred
\n
