Location Detail: NAT 1353 Gold Star Hwy Groton (10154) Shift Detail: Monday - Friday; 8:00am-4:30pm Program Summary Natchaug Hospital is Eastern Connecticut’s leading provider for children, adolescents and adults with mental illness and substance use addictions. The Adult Partial Hospitalization Treatment (PHP) and Intensive Outpatient Treatment (IOP) programs focus on a therapeutic environment where clients learn coping strategies and improve functioning. Treatment includes individual, group, and family therapy, and may involve up to six hours per day, five days per week. Job Summary Behavioral Health Clinician works with interdisciplinary teams to organize, prepare, and deliver evidence‑based clinical services in the Adult PHP. Responsibilities include individual, group, and family therapy. Key Responsibilities Conduct a comprehensive bio‑psychosocial assessment and establish a care plan as appropriate. Provide direct patient care, including case management, care coordination, interdisciplinary collaboration for care transition needs, crisis intervention/safety planning, counseling, emotional support, and referral and discharge planning. Refer patients to appropriate levels of care and resources that promote individualized care plans. Maintain timely, patient‑centered documentation that reflects assessment, interventions and outcomes in accordance with regulatory, licensing and third‑party agency requirements. Qualifications Master’s degree in social work, counseling, or marriage and family therapy, or a related human service field. Previous experience in a healthcare setting preferred, particularly with mental health and/or chemical dependency populations. Current Connecticut license: LMSW, LPC-A, LMFT-A, or CADC. Excellent communication skills and ability to establish relationships within and outside the organization. Awareness of health‑related social needs, cultural competence, and ability to recognize and address health disparities, inequalities, and injustices related to client access and service provision.
