Advancing Potential. Position Summary LADACIN Network is seeking a compassionate and dedicated Physical Therapist to join our Adult Day Program team. This part-time position provides physical therapy services to adults ages 21+ with developmental and multiple physical disabilities within a dynamic transdisciplinary environment. The ideal candidate will be passionate about maximizing independence, mobility, functional abilities, and quality of life through individualized, evidence-based therapeutic interventions. Key Responsibilities Provide evaluations, consultations, and physical therapy services to adult clients as scheduled. Develop, implement, and modify individualized treatment plans based on ongoing assessment and client progress. Participate in the development, implementation, and review of Individual Service Plan (ISP) goals and objectives. Maintain accurate and timely documentation, progress notes, reports, and electronic records. Collaborate with interdisciplinary team members, families, caregivers, and residential staff. Participate in transdisciplinary meetings, ISP meetings, and family conferences. Provide instruction, training, and support to staff regarding therapeutic techniques, positioning, mobility, transfers, and equipment use. Assist with the selection, adaptation, maintenance, and management of therapeutic equipment and supplies. Support clients in achieving mobility, functional independence, and community participation goals. Maintain compliance with all applicable professional, state, and agency standards. Participate in professional development, trainings, and continuing education activities. Required Qualifications Doctorate Degree in Physical Therapy (DPT). Current New Jersey Physical Therapist License in good standing. Strong interpersonal, organizational, and communication skills. Ability to work collaboratively within a transdisciplinary team environment. Ability to safely lift, transfer, and position clients as necessary. Must pass required background clearances, screenings, and drug testing. Preferred Qualifications Experience working with individuals with developmental and physical disabilities. Experience in adult services, rehabilitation, or community-based programs. Knowledge of adaptive equipment, positioning, mobility training, and functional independence programming.
