Key Responsibilities\n

• Patient Assessments: Conduct thorough and ongoing assessments to identify the unique mental health needs of each patient, ensuring comprehensive support tailored to their situations.

• Team Collaboration: Liaise effectively with supervisors, direct care staff, family members, and external stakeholders to discuss individual progress and therapeutic goals, facilitating an integrated approach to mental health care.

• Scheduling and Coordination: Develop and maintain a tentative schedule for patient appointments, home visits, and monitoring activities, ensuring everything aligns with patient care requirements.

• Program Implementation: Oversee and ensure the effective execution of treatment plans, coordinating home visits for in-home observations and staff interviews to gather valuable insight into patient progress.

• Documentation: Maintain meticulous records, including monthly and quarterly progress notes, to guarantee immediate availability of information for internal and external review.

• Training and Development: Organize and conduct training for staff members to ensure competency in implementing treatment plans and utilizing documentation systems optimally.

• Community Engagement: Act as a liaison between the clinic and the broader mental health community, fostering public awareness and support for development initiatives.

Qualifications\n
