Nurse Practitioner – Full Time (On-Site) $10k sign on bonus

Locations: Fairfield, CA

Schedule: Monday–Friday, flexible based on patient and partner needs

We work directly with local dialysis centers, nephrology offices, and patients in their homes to improve access to care, reduce avoidable hospital visits, and create a better experience for those managing long-term health conditions.

What You’ll Be Doing

• Deliver comprehensive, relationship-based care to adult and senior patients in a mix of clinic settings with an occasional in home and telehealth
• Conduct health assessments, screenings, and chronic condition management visits
• Develop and implement individualized care plans that address both clinical and lifestyle needs
• Partner with nephrologists, dialysis center staff, and care coordinators to ensure continuity of care
• Order and interpret diagnostic tests and communicate findings with patients and families
• Maintain accurate, compliant documentation in the EHR
• Participate in team discussions to drive quality outcomes and enhance the patient experience

What You Bring

• Current California Nurse Practitioner license
• Board certification (AANP or ANCC) in Family, Adult, or Geriatric Medicine
• At least 2 years of clinical NP experience in primary care, internal medicine, or chronic disease management (home care experience a plus)
• Strong understanding of complex and chronic condition care, including CKD and diabetes
• Active DEA and CPR/BLS certification
• Reliable transportation and valid California driver’s license
• Experience with value-based care or risk-adjustment documentation a plus

Compensation and Benefits

• Base salary: $150,000–$160,000 annually
