A growing healthcare organization is seeking Nurse Practitioners to partner with dialysis centers and care teams, supporting patients with complex kidney conditions in a value-based care model.

• Monday-Friday schedule no weekends
• Work with complex, high-acuity patients (CKD/ESKD)
• Collaborative model with dialysis teams and specialists
• Move beyond volume-driven care into purpose-driven medicine
• Mix of in-person care with some telehealth

• Provide primary care support for patients with chronic kidney disease
• Manage high-risk patients beyond basic wellness visits
• Play a key role in transitions of care and reducing readmissions
• Collaborate closely with dialysis and interdisciplinary teams
• Build meaningful, ongoing patient relationships

• Active California Nurse Practitioner license and Board Certification
• Experience in primary care, internal medicine, or complex care settings
• Comfortable managing high-risk or medically complex patients
• Interest in value-based care and outcomes-driven medicine
• Strong communicator who thrives in a team environment
