Physical Therapist - Home Health

Assignment Duration This is a 13-week contract position.

Schedule 8-hour shifts, Monday through Friday, 8 am to 4:30 pm.

Job Description We are seeking a dedicated Physical Therapist to provide quality care to patients in a home health setting. You will treat a range of conditions including general orthopedics, generalized weakness, strengthening exercises, and cardiac rehabilitation.

Responsibilities

• Evaluate and develop treatment plans for patients.
• Implement therapy interventions to improve patient mobility and functionality.
• Document patient progress in the EPIC charting system.
• Collaborate with a team of healthcare professionals to ensure comprehensive care.
• Manage a caseload of 5-6 patients per day, including admissions and discharges.

Required Experience / Certifications / Licensure

• Current Physical Therapist license for the state of West Virginia.
• BLS Certification.
• Positive attitude and willingness to learn the EPIC system.
