Registered Nurse - RN - Dialysis

Position Overview Home Therapy Registered Nurse – Paid one on one training and education provided. Empower, engage, and train dialysis patients on peritoneal and home hemodialysis using state‑of‑the‑art training programs and equipment. Primarily works in the clinic setting and conducts initial home visits with another staff member present. The position functions as an onsite Patient Educator and Case Manager for patients with various shifts and work hours. The Home Therapies RN CAP 2 may serve as an entry‑level designation within the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or who advance from RN CAP 1. Principal Duties and Responsibilities Accountable and responsible for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Delegate appropriate tasks to direct patient care staff (RNs, LVN/LPNs, Patient Care Technicians) as needed. Collaborate with the End Stage Kidney Disease (ESKD) health care team in decision‑making, teaching, leadership, and quality improvement activities. Perform ongoing, systematic collection and analysis of dialysis data, document findings, make treatment plan adjustments, and notify Supervisor or physician as required. Assess, collaborate, and document patient/family basic learning needs; provide initial and continuous education. Direct and provide, in collaboration with patient, home care partner, and clinical staff, all aspects of safe and effective delivery of dialysis therapies. Administer medications per prescription or approved algorithm, documenting justification and effectiveness. Initiate or assist with emergency response measures. Serve as a resource, leader, coach, mentor, and role model for new and incumbent employees. Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections. Identify expected outcomes, document and update nursing assessment and plan of care via collaboration with the Interdisciplinary Team. Ensure patient awareness of transplant and treatment modality options. Assist in identification, evaluation, selection, and education of Home Dialysis candidates and Home Partners. Assess barriers in candidates’ home environments and partner readiness. Train home dialysis patients and Home Partners on safe, effective operation and maintenance of all equipment and supplies through a formalized training program. Participate in education and quality improvement projects; may serve as a preceptor to new employees. Complete CAP requirements to maintain or advance within the program. Perform all other duties assigned by Supervisor. Physical Demands and Working Conditions The position requires direct patient care that involves heavy lifting, moving patients, and assisting with ambulation. Frequent, prolonged periods of standing and bending are required. The employee may move machines and equipment up to 200 lbs. and lift chemical and water solutions up to 30 lbs. at heights up to 5 feet. The work environment is a health care facility with controlled temperature and moderate noise levels. Exposure to infectious materials is possible. The position includes on‑call rotation, night, weekend, holiday, or as defined by program needs. Travel to training sites, other facilities, and patient homes may be required. Desk and computer work, as well as interaction with patients, staff, and physicians, constitutes part of daily duties. Supervision Assists in providing oversight of RNs, LPNs/LVNs, Patient Care Technicians, and Home Therapies Care Team Assistants as a Team Leader or designated Nurse in Charge after meeting all other prepared training and experience requirements. Minimum of 3 months experience in home dialysis therapies is required. Education, Licensure, & Certifications Graduate of an accredited School of Nursing. Current state licensure appropriate for the state of employment. Current CPR/BLS Certification or successful completion of CPR/BLS. Meet practice requirements in the employed state. Experience and Required Skills Entry level for RNs with a minimum of 2 years of Nephrology Nursing experience within the last 2 years. Working knowledge of home dialysis therapies and patient education. Strong leadership, communication, and teamwork skills.
