Registered Nurse - Home Health

Job Description

Job Summary:
The Home Health Registered Nurse provides skilled nursing care to geriatric patients in their homes, ensuring high-quality clinical assessments and interventions. The role involves maintaining compliance with documentation standards and collaborating with interdisciplinary teams to deliver patient-centered care.

Responsibilities:

• Assess and document patient conditions using OASIS standards to develop and update individualized care plans.
• Administer wound care, pressure ulcer management, and operate wound VACs per clinician orders.
• Manage IV therapy, central lines, ports, and perform blood draws, medication administration, and infusion treatments.
• Provide education on medication, pain management, nutrition, and use of medical devices such as G-tubes and urinary catheters.
• Document visits and clinical notes accurately within 24-48 hours to ensure compliance with agency requirements.
• Collaborate with team members to coordinate patient care and address urgent clinical needs, supporting facility scope of practice when necessary.

Qualifications:

• Current Maryland or compact RN license required.
• 2-3 years of recent skilled home care experience, including OASIS documentation.
• Proficient in wound care, infusion therapy, and complex wound/vital management.
• Valid driver's license and reliable vehicle required for community visits.
• BLS certification required.
• Strong clinical and assessment skills with experience in home health admissions and recertifications.
