Our organization operates Skilled Nursing Facilities (SNFs) that offer:
- Long-term care: Support for residents needing ongoing skilled assistance and compassionate daily care.
- Short-term rehabilitation: Services for patients recovering from surgery, illness, or injury.
- Collaborative care: An interdisciplinary team approach involving nursing, therapy, and medical staff working in unison.
- Diverse patient population: Care for individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs.
As a therapy professional, you will play a vital role in helping patients enhance their functional abilities and improve their quality of life in a supportive, patient-focused setting.
Position SummaryThe Speech Language Pathologist (SLP) delivers direct rehabilitation services in speech therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.
Qualifications and Experience- Graduation from an accredited speech therapy program (minimum MS or MA in SLP)
- ASHA certification (CCCs) or eligibility for a 9-month Clinical Fellowship Year (CFY) to obtain ASHA certification; must maintain ASHA credentials once achieved.
- Current registration/licensure in the applicable state, with an active license in good standing throughout employment.
- Preferred one year of experience; however, new graduates are welcome to apply.
- CPR certification required upon hire or to be obtained during orientation; must remain current during employment.
- Engage in community health initiatives as appropriate.
- Proficient in Microsoft Word, Excel, and email communication.
- Coordinate with patients, families, and support departments to effectively address patient needs.
- Exhibit proper body mechanics at all times.
- Demonstrate the ability to make independent decisions when necessary.
- Familiarity with practices, procedures, and regulations governing post-acute care facilities.
- Implement and interpret departmental programs, goals, policies, and procedures.
- Maintain proficiency in all competency areas, including patient rights, safety, and sanitation.
- Foster professional relationships with colleagues, vendors, and others.
- Ensure confidentiality of all proprietary and sensitive information.
- Adhere to company policies, including those related to harassment and compliance.
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training.
- Establish, evaluate, and adjust realistic, measurable, and functional goals.
- (SLP CCC only) Oversee and assess care provided by SLP CFYs.
- (SLP CFY only) Follow speech therapy treatment plans under the supervision of an SLP CCC, in accordance with federal and state guidelines.
- Document patient progress accurately and in a timely manner.
- Utilize therapy software effectively and accurately.
- Provide excellent customer service and maintain a positive attitude towards patients.
- Assist in the safe evacuation of patients when necessary.
- Demonstrate reliable attendance and punctuality.
- Employ reasoning skills and sound judgment in decision-making.
- Communicate effectively and collaborate within an interdisciplinary team.
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, walking, and reaching throughout the workday.
- Read, write, speak, and comprehend the English language proficiently.
